Youth with Special Health Care Needs in Transition Formative Research Report

This report for presents the findings of an innovative, qualitative study exploring the experience of youth with special health care needs as they transition into adulthood. It focuses on the importance of including youth in the decision-making process, and uses their insights and experi­ences to help shape and inform recommen­dations for future initiatives. It includes background information, an environmental scan, as well as research findings and quotes from study participants.
