Federal Activities Related to the Medical Home Model for the Health Resources and Services Administration

New Editions supports the HHS New Freedom Initiative Working Group, led by the HHS Office on Disability. The Health Resources and Services Administration (HRSA) is working through the New Freedom Initiative Working Group on a project to analyze and promote the Medical Home framework to services for children with special health care needs and all individuals with disabilities. We provide conference management, notetaking and administrative support for the Working Group. We completed an analysis of over 45 HHS programs and activities related to children with special health care needs in the context of Medical Home model of service delivery to identify existing programs in service, policy and research arenas, gaps, potential collaborations, and action steps for change.
